We partner with an electronic medical records system (or EMR) called Prompt. This system allows you to schedule appointments at your convenience, set your preferred appointment days and times and provide payment online. For new patients with a referral

Once we receive a referral from your healthcare provider, we’ll reach out to you by email and/or text message with a link to schedule an evaluation appointment.

For new patients without a referral

To schedule an evaluation appointment without a referral from a doctor, please submit the form below and we’ll follow up with a link to schedule your evaluation appointment.
